You should have now received a letter about this from NHS England. We understand it may be a difficult for you, as you may have been registered here as a patient for many years; however, we strongly encourage you to start looking for a new GP practice now and will support you with this.


How to find a new GP practice


In the letter were details of all the GP practices within 3 miles of Myddleton Road Surgery. You can also find a new practice by using the ‘find a GP’ search function on the homepage of the NHS website: You can input your own postcode to find the practices nearest to your home.

Registration form


When you have chosen a new practice, you will need to complete a family doctor services registration form. This form is also called a ‘GMS 1’ form.


You can collect a form from Myddleton Road Surgery and complete it before taking it to your new practice. The practice staff will be able to support you if you need help completing the form.


You can also get a GMS 1 form from the new practice or by downloading it from the website link below:


You will need to provide your name, address, date of birth and NHS Number (if known) on the form.


Other information that the new practice may ask for


GP practices may also ask to see proof of your address, such as a recent utility bill (gas, electricity, water or phone bill, but not a mobile phone bill) or a council tax bill.


This information is requested to check that you live within the practice catchment area and to ensure they have the correct spelling of your name and address to add to their clinical system.


However, if you do not have all of this information, the practice cannot refuse to register you.  


How will I know if I am registered with my new practice?


The GP practice will send your registration form to the London patient registration services team, who will write to you confirming that you are registered with the new GP practice.  

Please note, that once you have registered, you will not be able to see the clinicians at Myddleton Road Surgery.


What will happen to my medical records?


Your medical records will be transferred to the new practice automatically. This will only happen when you register with your new practice.

Appointments are now available for all patients registered at any Haringey practice.

Monday to Friday: 6:30 – 8:30pm

Saturday and Sunday: 8:00am to 8:00pm

You can make an appointment if you are registered at any practice in Haringey. You will be able to see a local Haringey GP or healthcare professional. It may be someone from your practice, and if not, it will be another local GP, Practice Nurse or Healthcare Assistant.


Speak to your local pharmacist about stocking up on your medicines cabinet to treat common conditions for you and your family

The following medications can be purchased from pharmacies or your local supermarket and are generally cheaper than buying them on prescription.

    Allergy medicines

    Simple pain killers like paracetamol and ibuprofen

    Sore throat, coughs, colds and flu medications

    Heartburn and indigestion remedies

    Pile (haemorrhoid) treatments

    Anti-diarrhoea medication

    Rehydration salts

    First aid kit including plasters and bandages



By keeping a selection of essential medications at home you can treat common conditions in a timely manner, avoid unnecessary trips to see your doctor and/or even visits to the A&E department.



Community pharmacists have the knowledge and skills to help and advise you on treating common conditions, how to use the medicines you have at home and local services that are available to you.

There are many pharmacies across Haringey. They can be found in local communities, on the high street, in some supermarkets and in shopping centres.

You do not need to make an appointment to see the pharmacist, just pop in. They are always happy to help and answer any questions you have.

Many pharmacies are open late nights and at the weekend, which is convenient for people who work and if you or any member of your family feels unwell when the doctor’s surgery is closed.
